BASIC LIFE INSURANCE AND AD&D - Provided at no cost to you
Basic life Insurance and AD&D provides financial protection for you and your family in the event of your death. CHL provides $40,000 of life insurance and AD&D coverage for each eligible employee. This plan is administered by Symetra.

LONG-TERM DISABILITY - Provided at no cost to you
The long-term disability program begins when you are disabled 6 months. CHL provides long-term disability insurance for each eligible employee. The benefit is 60% of monthly wages with a maximum of $5,000 per month. This plan is administered by Symetra and is provided at no cost to you.

ADDITIONAL LIFE INSURANCE - This is an optional benefit you may purchase
You have the option of purchasing additional coverage for yourself, spouse and children. You can purchase life insurance for yourself starting at $10,000 in increments of $10,000, up to $500,000 (not to exceed 5x your earnings). Coverage for your spouse can be elected in $5,000 increments, not to exceed 100% of your election, up to $250,000. Coverage for your child(ren) is available up to $10,000. Medical underwriting may be required. This plan is administered by Symetra.

SHORT-TERM DISABILITY - This is an optional benefit you may purchase
The Short-Term Disability program provides financial protection for you by paying a portion of your income when you are Disabled. The benefit is in the amount of 66 2/3% of your salary with a maximum of $1,000, or $2,000 per week if your salary is over $80,000/year. This plan is administered by Symetra.
